Postmortem:
On September 23rd 2021 at 16:21 UTC our engineers identified an issue that caused unexpected behavior with the creation process of Surveys, if a Survey Template was used.

What happened?
If a user began creating a new survey based on a Survey Template, but then closed out of the creation process without saving the new survey, the Survey Template settings would unexpected be pre-populated the next time they began creating a new survey without using the Survey Template.

What will we do to prevent this from happening in the future?
We will hold an internal postmortem to establish any relevant follow-up actions.
